About this deal

MEDIHONEY calcium alginate dressings can be safely worn for up to seven days. Change the dressing as soon as the secondary bandage reaches its absorbent capacity, or as prescribed by your loved one’s primary care physician. Many of the studies included in these SRs did not report on the type of honey that was used. For those that did report, the honey ranged from fresh, unaltered honey to irradiated medical grade honey (e.g., MediHoney). This makes it difficult to generalize the results to the Canadian context and to the types of medical grade honey that are available for use by Canadian practitioners. Further research, including properly blinded RCTs, is likely necessary to help to support the use of honey for wounds as a part of standard practice. There are a variety of types of wounds, acute wounds (including cuts, scrapes, burns, trauma, needle punctures, and surgical incisions acquired in healthcare settings) and chronic wounds (diabetic foot ulcers or pressure ulcers). 1 The management of wounds presents a burden on the Canadian healthcare system. In 2011 to 2012, more than 2.6 million wounds were treated in Canadian healthcare settings. 1 In 2013, it was estimated that infected pressure ulcers and surgical wounds cost individual hospitals around one million dollars a year. 1

MEDIHONEY looks similar in appearance and texture to traditional store-bought honey, but it’s not a sweetener. Instead, it provides an all-natural way to treat open wounds, preventing gangrene and infection. Often, wounds that have progressed carry a malodorous smell. MEDIHONEY has also been shown to decrease odors that may accompany wounds. Until recent years, the use of honey as an occlusive dressing for wounds and burns was considered a natural, but unproven, alternative to traditional wound management products. Today, researchers have caught up with ancient wisdom and the use of medical-grade honey for the treatment of wounds is supported by a growing body of scientific evidence. According to a literature review published in the journal Wounds, honey offers the following benefits in healing wounds:

Jull et al. 15 found that, in one trial, there was no significant difference in healing between honey and saline gauze at 16 weeks or in negative wound swabs at four weeks. 15 In another trial, the mean time to surgical closure was not significantly different between honey and povidone-iodine dressing but it was unclear whether the wounds actually healed. 15 Adverse events were not described well enough to be reported in the SR. 15 The authors of the HTA identified an additional RCT of Manuka-honey dressing versus conventional dressing and found there was no statistically significant difference in the mean duration of healing. 12 There was not enough evidence to comment on honey’s effect on infection in diabetic foot ulcer. 12 In addition to its antibacterial properties, medical honey hastens the healing of wounds through its anti-inflammatory effects. The amount of wound exudate is related to the activity of the local inflammatory process, in particular in wounds, which are colonized or infected with bacteria. Thus, the anti-inflammatory action of honey reduces oedema and the amount of exudate by down regulating the inflammatory process. It also reduces pain, as the pain in wounds results from the nerve endings being sensitized by prostaglandins produced in the process of inflammation, as well from the pressure on tissues resulting from oedema. There was overlap between the studies included in the HTA 12 and the SR by Jull et al. 15 as the HTA included an earlier version of the SR by Jull et al. in their analysis. No other SRs included in the CADTH report were included in the HTA.

Honey may contain spores from Clostridium botulinum, which are inactivated. The ready to use product is delivered sterile. Thoroughly clean the wound with a saline solution. (Here’s a recipe from IntermountainHealthcare). Then, if necessary, apply a barrier cream around the edges of the wound. Barrier cream prevents skin breakdown and infection. Honey is a sugary, syrupy substance that has been shown to have bioactive components that can help heal wounds. Robson, V., Dodd, S and Thomas, S. Kateel et al. 14 described their literature search as “exhaustive” but listed a limited number of key words that were searched in the literature. The authors also did not specify how study selection and data extraction were performed. 14 There was no list of excluded studies provided in three SRs. 13 , 14 , 16 The HTA 12 and four of the five SRs 13 , 15 – 17 reported comprehensive literature search strategies and study selection and data extraction was performed in 12 , 13 , 15 , 16 , 17 Kateelet al, 14 did not specify how their literature search or selection and extraction were performed. The included SRs reported primary study and patient characteristics, with the exception of Vandamme et al. 17 who did not report patient characteristics. 17

What is MEDIHONEY used for?

Honey is one of the few nutritive substances that does not spoil. In fact, still-edible honey has been discovered in 2,000-year-old sealed pots safely nestled within the final resting places of Egyptian pharaohs. There is a growing body of research evidence that supports the use of medical-grade honey as a top choice in advanced wound care treatment options. Healthcare professionals are increasingly using this medicinal substance in their practice. The anti-inflammatory action of honey has been extensively observed clinically ( 19, 28, 29) and in animal models ( 15). Animals do not demonstrate any placebo effects, as they are incapable of having attitudes influence their healing process, such as believing that natural products would be more effective, or from hearing via the news media of the effectiveness of honey in wound treatment ( 30). Thus, these observational and histological studies can be cited as convincing evidence for the positive results with honey not being due to a placebo effect. Similarly, this can be concluded also for the observations in wounds in animal models of honey stimulating the rate of angiogenesis, granulation and epithelialization ( 31, 32), which would explain the findings in clinical trials that honey speeds up the healing process ( 15).

Some patients (5 out of 100) experience pain after the application of medical honey to the wound. In some of them, treatment with medical honey has to be stopped. Few patients with pre-existing atopic diseases show local atopic reactions. No severe systemic atopic reaction due to medical honey has been reported in the literature. Medical honey has to be kept in the wound for 12–24h a day. Thus, it is combined with particular dressings like calcium alginates of hydrofiber dressings, which add substantially to the overall cost of treatment. Depending on the amount of exudate, the dressing with medical honey has to be changed up to 2 times a day in acute inflammatory wounds. The most often practised change interval for medical honey dressings is every 24–48h. This document is prepared and intended for use in the context of the Canadian health care system. The use of this document outside of Canada is done so at the user’s own risk. Medical-honey dressings are easy to use and do not cause trauma to the skin during removal. Another mechanism may be related to the pH level of honey being low (3.4–5.5; mean 4.4) ( 35, 36). Bacterial colonization or infection and recalcitrant wound healing situations are often accompanied by pH values >7.3 in wound exudates ( 37, 38). It has been demonstrated that acidification of wounds speeds healing ( 39), this being attributed to the low pH increasing the amount of oxygen off-loaded from hemoglobin in the capillaries. More recently it has also been attributed to suppression of protease activity in wounds by getting away from the neutral pH that is the optimum for their activity ( 37). This protease activity results from excessive inflammation. The anti-inflammatory activity of honey would thus remove this impediment to healing, as would the antibacterial activity working through removing infecting bacteria stimulating the inflammatory response.
